Transitional Program
A Fresh Start Rooted in Faith, Hope, and Healing
At Penfield Addiction Ministries, we believe recovery is a journey that continues long after the first steps toward healing. Our Transitional Program is designed to support individuals who are stepping down from higher levels of care or seeking a structured, affordable path forward in recovery.
In a safe, Christ-centered environment, residents continue building the skills needed for lasting sobriety while gradually transitioning back into everyday life. Through ongoing accountability, therapeutic support, and spiritual growth, the program helps individuals strengthen their foundation, gain confidence, and move toward greater independence with hope and purpose.
Holistic Healing
We focus on healing the whole person—body, mind, and spirit—through evidence-based treatment, counseling, and spiritual support.
Structured Daily Schedule
Each day is intentionally designed with therapy, education, recreation, and reflection, helping you develop healthy routines that last beyond treatment.
Personal Growth & Progress
With guidance from our experienced team, you’ll build coping skills, strengthen relationships, and prepare for life after treatment.
Inside the Transitional Program
-
The Transitional Program provides a structured yet flexible environment where residents begin applying recovery principles in real-world settings. Clients live in a therapeutic community that emphasizes accountability, peer support, and personal responsibility while gradually increasing independence.
Residents are expected to actively participate in program activities, maintain sobriety, pursue employment, and engage in both clinical and spiritual growth. This phase bridges the gap between intensive treatment and independent living, allowing individuals to practice recovery in daily life with ongoing support.
-
The Transitional Program offers a combination of clinical, spiritual, and practical support services, including:
Psychoeducation to build understanding of addiction, relapse prevention, and recovery skills
Group therapy to process experiences, develop insight, and strengthen interpersonal skills
Individual counseling to address personal goals, challenges, and continued growth
Participation in 12-step meetings and recovery-focused community support
Church services and spiritual development opportunities
Life skills training, including employment readiness, time management, and responsibility
Support in obtaining and maintaining employment
A structured, therapeutic living environment that promotes accountability and community
-
This program is ideal for individuals who:
Are stepping down from residential or PHP levels of care and need continued structure
May not require or have access to higher levels of care but still need a supportive recovery environment
Are ready to begin working, rebuilding independence, and reintegrating into daily life
Benefit from ongoing clinical support while practicing recovery outside of intensive treatment
The Transitional Program serves as a critical step in helping individuals solidify recovery skills while navigating real-world responsibilities.
-
Length of stay is flexible and based on individual needs, progress, and readiness for independent living. Most residents participate for several months, allowing time to establish stability in sobriety, employment, and daily routines before transitioning to a more independent level of care or supportive living environment.
